Patient Rights and Responsibilities Std. 7.01

Every family who works with Aspire Behavioral Academy — current or prospective — has the following rights and shares the following responsibilities. A printable version is provided at intake and is available on request at any time.

You have the right to…

    Respectful, dignified careregardless of race, ethnicity, national origin, language, religion, gender identity, sexual orientation, ability, or family structure.

    Clear informationabout your child's assessment, diagnosis (where applicable), treatment plan, expected outcomes, and progress — communicated in plain language and your preferred language whenever possible.
  • 03
    Informed consentbefore any assessment, treatment plan, or change in services begins.
  • 04
    Participationin goal-setting, treatment planning, and ongoing decisions about your child's care.
  • 05
    Privacy and confidentialityconsistent with HIPAA and our Notice of Privacy Practices.

    Access to your child's recordsand the ability to request corrections.

    Share complete informationabout your child's medical, behavioral, and developmental history so we can plan effectively.

    Communicate openlywith your child's clinical team — ask questions, raise concerns, tell us what isn't working.

    Attend caregiver-training and team meetingswhen scheduled, or let us know when you cannot.

    Provide reasonable noticewhen canceling sessions so coverage can be reassigned.

    Maintain a safe environmentfor sessions delivered in your home.

Grievance & Feedback Process

Concerns can be raised in person, by phone, or in writing to the Clinical Director. We acknowledge grievances within two business days and respond in writing within fourteen days.

BACB Ethics Code Adherence

All clinical staff at Aspire Behavioral Academy adhere to the Behavior Analyst Certification Board (BACB) Ethics Code for Behavior Analysts. Concerns regarding clinician conduct may be reported internally or directly to the BACB.
