This notice describes how medical information about your child may be used and disclosed, and how you can get access to that information. Please review it carefully.
Our responsibility
Aspire Behavioral Academy is required by law to protect the privacy of your child’s protected health information (PHI), to give you this notice describing our privacy practices, and to follow the terms of the notice currently in effect. We are committed to safeguarding the confidentiality, integrity, and availability of all patient information, consistent with HIPAA and our contractual obligations.
How we use and disclose health information
Without needing separate written authorization, we may use and disclose your child’s PHI to:
- Provide treatment — so our behavior analysts and technicians can assess, plan, deliver, and coordinate your child’s care, including sharing information with other providers involved in that care.
- Obtain payment — to verify coverage, obtain authorization, submit claims, and follow up on billing with your health plan.
- Run our operations — for quality improvement, staff training and supervision, internal auditing, accreditation, and business management.
- Meet legal obligations — where required by law, including mandated reporting of suspected abuse or neglect, public health requirements, and responses to lawful requests from regulators or courts.
When we need your permission
For any purpose other than those listed above, we will ask for your written authorization before using or sharing your child’s health information. This includes the use of photographs, video, testimonials, or commentary for marketing, training, or instructional purposes. You may withdraw an authorization at any time, in writing, without penalty; we will honour and action that withdrawal promptly. Withdrawal does not apply to information already shared while the authorization was in force.
Our staff are strictly prohibited from creating or sharing any written, photographic, or video content that could identify a current or former client in any social media context.
Your rights over your child’s health information
You have the right to:
- See and receive a copy of your child’s health records.
- Request corrections or amendments to that information.
- Be told how your child’s health information is used and shared.
- Ask us to limit how we use or share the information, including requesting restrictions on particular disclosures.
- Receive a report of when and why your child’s health information was shared.
- File a complaint if you believe your privacy rights have been denied or your information has not been protected.
How we protect the information
Electronic records are secured through encryption, password protection, firewalls, secure backups, and role-based access controls, so staff see only the records they are authorized to see. Information in transit — including telehealth sessions, email, and portals — is encrypted and transmitted through secure platforms. Physical records are stored in locked cabinets in restricted-access areas.
How to raise a concern or file a complaint
If you believe your privacy rights have been violated, you may contact us at info@aspirebehavioralacademy.com or +1 (508) 276-5040. We will begin an impartial review and respond with our findings. You will never be penalized, retaliated against, or have your family’s access to services affected for raising a concern.
You may also file a complaint directly with the U.S. Department of Health and Human Services, Office for Civil Rights, at hhs.gov/ocr/complaints, or with your health plan.
Changes to this notice
We may update this notice. When we make a material change, we will publish the revised notice on this page and reissue it to families currently receiving services. The version in force is shown at the top of this page, and the version you acknowledged is recorded with your intake.
Receiving this notice
We provide this notice to every family before services begin and keep a signed acknowledgement of receipt in your child’s file. You can ask us for another copy at any time, and this page is always available at https://aspirebehavioralacademy.com/notice-of-privacy-practices.
