What Personal Data Do We Collect?

Demographic Data

Parents or guardians may also provide information about a child, such as date of birth, gender, school, and address, where it is needed to assess and deliver care.
Insurance and Billing Data
The Site does not take payments. Through the intake form we may collect insurance details — such as your plan, member ID, group number, and images of your insurance card — so that we can verify coverage and bill your plan for services. Any payments you make to us are handled outside this Site.
Support Data
If a User contacts us for support or to lodge a complaint, we may collect technical or other information from log files and similar technologies, some of which may qualify as Personal Data, such as an IP address. We use this information for troubleshooting, customer support, software updates, and service improvement. If we plan to share Personal Data with a third party other than as described here, we will provide notice at the time of collection or transfer when required and provide available choices regarding the transfer.
How Long Do We Retain Personal Data?
We retain Personal Data for as long as we are providing services to your family, and afterwards for the period required by applicable healthcare record-retention law. The actual retention period depends on the type of information, contractual commitments, and applicable law. We keep information only as long as necessary for the purpose for which it was collected, unless longer retention is required or permitted for a legitimate business purpose. At the end of an applicable retention period, we will remove information from our databases and ask relevant service providers to remove it from theirs where appropriate. We do not use cookies to retrieve information unrelated to your use of the Site.

We are committed to protecting the security and confidentiality of Personal Data. We use a combination of reasonable physical, technical, and administrative safeguards intended to protect information in our possession or control from unauthorized access, use, disclosure, alteration, or destruction. Information Submitted by Minors
If a child is under 13, a parent or legal guardian must provide consent before allowing the child to use the Site where applicable.
